Your personal code
Apply using the signup form on this page, or email your name and email address to AOX4. We review requests and email your personal code and sharing link. Codes are not issued automatically. We use your information to manage your application and rewards; marketing emails are optional.
What qualifies
- Members must be 18 or older. No purchase is required to join.
- Your friend must be a new AOX4 customer with no previous orders and must use your assigned code at checkout.
- Their merchandise subtotal must be at least $30 before the 15% discount. Shipping and taxes do not count toward the minimum and are not discounted.
- The friend offer is one use per customer and cannot be combined with other discount codes.
- No self-referrals, duplicate accounts or fabricated orders. UAMS sample code SCRUBS15 does not award an individual referral reward.
When you earn credit
Each qualifying first order earns one $10 store-credit reward, regardless of how many tubs it contains. We review the order after 30 days from delivery and after any return or payment dispute is resolved. Canceled or fully refunded orders do not qualify. A partial return must leave at least $30 in qualifying merchandise before discounts.
Rewards are reviewed and issued manually. We email you when credit is available. Credits accumulate in your AOX4 customer account and have no cash value. Use the same email for signup and your customer account. Contact us to check your balance or get help redeeming it. We do not issue duplicate credit for the same order.
Sharing your referral
When sharing your link or code, clearly explain that you can earn store credit, for example: “You get 15% off your first qualifying order, and I earn $10 in store credit.” Share your own experience without medical claims or suggestions that AOX4 improves patient care. Follow your employer's rules. AOX4 is not endorsed by UAMS or any hospital.
Photo-review reward
Receive one $10-off coupon for an original photo of your AOX4 product and an honest review based on your experience. All star ratings and opinions qualify equally. Scrubs are optional. Customers and sample recipients may participate; tell us if you received the product free. One photo-review reward per person. The coupon is valid for 90 days after issue on a future merchandise order of $30 or more before discounts, excludes shipping and taxes, and cannot combine with other discount codes. This coupon is separate from referral store credit.
While website photo reviews are being set up, email your photo, review and order number (or identify yourself as a sample recipient) to rrennat16@yahoo.com. Include: “I was offered a discount for sharing my honest review.” Also disclose a free sample if applicable. Negative feedback is eligible. Do not include patients, confidential information or other people without their permission. We ask separately before using your photo in advertising.
